About 8% of the population suffer from fistulas - purulent channels that connect an internal opening in the anus and the outer skin around it. An innovative treatment with a biological glue produced from blood plasma enables a short and relatively simple treatment of this painful problem.

Around the anus are glands that permanently secrete a lubricant. In some cases, an infection may develop in these glands that blocks the gland and creates an abscess (abscess). If the abscess drains outward, into the skin of the buttock, a fistula is formed.

Prof. Amram Ilon, director of the department of general surgery and transplants at Sheba Hospital, explains that the more superficial fistulas, which do not pass through the external sphincter muscle, or pass through a small part of this muscle, can be treated relatively simply, by cutting the skin tissue between the anus and The opening of the fistula. In this way, the canal is open to the air and heals within a few weeks. According to Prof. Ayalon, this method is very effective and cures with a high rate of success. But in cases where the fistula is deeper, it cannot be used. Cutting the skin tissue in the same way in a deep fistula, which passes over or through a significant part of the external sphincter muscle, may damage the sphincter muscle and the ability to control it. For this reason, until recently the treatment of deep fistulas involved a complicated operation with a moderate success rate in the long term.

Innovative treatment with fibrin glue makes it possible to avoid surgery. Dr. Oded Zamora, the initiator of the use of this therapeutic method at Sheba, explains that fibrin is a biological glue that is produced from the blood and mimics the body's natural coagulation mechanism. When it is inserted into the fistula, by injection, a fibrin clot is formed that closes the fistula for a few weeks and then disintegrates. Dr. Zamora explains that the formation of the clot in the fistula encourages the body's self-healing and growth processes. Later, the clot will naturally be replaced by a scar that will close the fistula and lead to its healing.

The main advantage of this treatment method, according to Dr. Zamora, is that it involves a relatively simple surgical operation. The treatment is a short process, which does not involve cuts and blood loss and allows for a quick recovery and a return to normal activity on the same day or the next day. Dr. Zamora says that the success rates of treatment with the first injection are 60-50%. If the problem is not solved, you can go back and inject the fibrin glue a second time; The chances of success of the treatment increase from injection to injection. Only if the treatment is ineffective should one resort to surgery.
